ABSTRACT

Abstract Lesions of non-endodontic origin may mimic periapical abscess. Osteosarcoma is a rare malignant lesion. Case report The present report describes a case of chondroblastic osteosarcoma in the periapical region of teeth #29, #30, and #31 of an 18-year-old male. Clinical history showed self-reported discomfort in the right posterior gingiva for over a month. Physical examination showed a small expansion and redness of the right mandibular buccal and lingual cortical plates, but no signs of pain or inflammation were observed. All the teeth responded positively to pulp sensibility. Periapical and panoramic radiographs showed slight periapical radiolucency in the roots of teeth #29 and #30, clear periodontal ligament space widening, and evident loss of lamina dura. Incisional biopsy was performed, and based on microscopic findings the diagnosis of chondroblastic osteosarcoma was confirmed. Conclusions Non-endodontic diseases associated with tooth root apex, such as chondroblastic osteosarcoma, should be included in differential diagnosis of jaw lesions that resemble periapical abscess.

ABSTRACT

El uso de la tomografía computarizada de haz cónico (CBCT) en endodoncia ha ido en aumento, justificando su uso en casos más complejos, como cirugías periapicales. El objetivo fue evaluar las características imagenológicas encontradas en un grupo de pacientes derivados a cirugía periapical, mediante el uso de CBCT. Se realizó un estudio observacional descriptivo, en un grupo de pacientes, derivados a cirugía periapical, a los cuales se realizó un examen de CBCT, evaluación clínica y radiografía periapical. Un total de 18 pacientes fueron examinados (6 hombres y 12 mujeres), cuyas edades estaban entre los 19 y 64 años de edad. El diámetro mayor de las lesiones varió entre 6 mm a 16 mm. El uso de CBCT en cirugías periapicales entrega información más completa al cirujano, ya que permite mayor precisión al realizar el acceso quirúrgico y detecta un mayor número de lesiones periapicales que con la radiografía periapical, logrando en casos complejos un mejor diagnóstico y planificación del tratamiento.


The use of cone beam computed tomography (CBCT) in endodontics has increased, justifying its use in more complex cases, such as apical surgery. The aim was to evaluate the radiologic characteristics found in a group of patients referred to apical surgery, using CBCT. A descriptive study was conducted in a group of patients referred for apical surgery. In these patients, a clinical examination, a conventional radiographic evaluation and CBCT were performed. A total of 18 patients were examined (6 men and 12 women), ages were between 19 and 64 years old. The major diameter of the lesions ranged from 6 mm to 16 mm. The use of CBCT in apical surgery provides more complete information to the surgeon, allowing greater precision when performing surgical access and detects a greater number of periapical lesions than with conventional radiography. In complex cases is possible obtain better diagnosis and treatment planning.

ABSTRACT

Abstract Interleukin 17A (IL-17A) is a proinflammatory cytokine responsible for the initiation and propagation of inflammation. One of its actions is the recruitment of neutrophils to the site of infection. The aim of this study was to investigate whether there is association between IL-17A expression and neutrophil infiltration in periapical abscesses and periapical granulomas, as well as to find which type of T lymphocyte effector (CD4+ or CD8+) expresses IL-17A in these lesions. Elastase, CD4, CD8, and IL-17A were analyzed by immunohistochemistry and immunofluorescence, in the biopsies of periapical lesions. Abscess lesions exhibited the highest labeling area for IL-17A (p = 0.011). During double immunofluorescence staining, there were significantly more CD4+/IL-17A+ cells compared to CD8+/IL-17A+ cells, both in the abscesses (p = 0.025) and granulomas (p = 0.011). In conclusion, IL-17A was intensively expressed in periapical abscesses rich in neutrophils. The high percentage of IL-17A in these cases suggests the participation of this cytokine particularly in the acute stages of the inflammatory process of the periapical lesions.

ABSTRACT

Often there is the need of moving teeth endodontically treated or teeth still in endodontic treatment. In order to collaborate with the comprehension and substantiation of the following subjects will be discussed: 1) Orthodontic movement in endodontically treated teeth without periapical lesion, 2) Orthodontic movement in endodontically treated teeth with inflammatory periapical lesion, and 3) Orthodontic movement in teeth endodontically treated due to aseptic pulp necrosis by dental trauma. In practically all situations, endodontically treated teeth to be orthodontically moved must be subjected to a careful evaluation by the endodontist about the conditions, adequate or not, of the endodontic treatment. Then, in this paper it was sought to induce an insight for new clinical researches about the theme that may definitely prove the information obtained by interrelations of information in parallel to clinical practice.


Com frequência, há a necessidade de movimentar-se dentes endodonticamente tratados ou ainda em tratamento endodôntico. Para colaborar com a compreensão e fundamentação do assunto, abordaremos: 1) a movimentação ortodôntica em dentes endodonticamente tratados sem lesão periapical; 2) a movimentação ortodôntica em dentes endodonticamente tratados com lesão periapical inflamatória; e 3) a movimentação ortodôntica em dentes endodonticamente tratados de necrose pulpar asséptica por traumatismo dentário. Em praticamente todas as situações, dentes endodonticamente tratados a serem movimentados ortodonticamente devem ser submetidos a uma avaliação criteriosa, por parte do endodontista, sobre as condições adequadas ou não do tratamento endodôntico. Em seguida, no presente trabalho procurou-se, singelamente, induzir um insight para novas pesquisas clínicas sobre o tema, que possam definitivamente comprovar os conhecimentos obtidos pela inter-relação de conhecimentos em paralelo à prática clínica.

ABSTRACT

Periapical surgery aims to remove periapical pathology to achieve complete wound healing and regeneration of bone and periodontal tissue. Platelet rich fibrin (PRF) is a wonderful tissue engineering product and has gained much popularity due its promising results in wound healing bone induction. The features of this product are an attribute of platelet cells, which, after cellular interactions, release growth factors. This case report illustrates the use of PRF in bony regeneration after enucleation of cyst in anterior maxilla.

ABSTRACT

Periradicular surgery aims to achieve complete wound healing and regeneration of the periodontal unit. Platelet-rich plasma (PRP) is a platelet concentrate that is widely used to promote tissue healing, and bone induction through its various growth factors has been used in this study. This case report describes the use of PRP unilaterally in a bilateral periapical lesion in the same patient. The site treated with PRP showed better healing than the untreated site. This was confirmed with computed tomography (CT) scan readings. It was hypothesized that PRP could promote tissue regeneration and alveolar bone repair.

ABSTRACT

The esthetics and functional integrity of the periodontal tissue may be compromised by dental loss. Immediate implants became a viable option to maintain the periodontal architecture because of their anatomic compatibility with the dental socket and the possibility of eliminating local contamination. This article describes the procedure of immediate implant placement in the anterior maxilla replacing teeth with chronic periapical lesions, which were condemned due to endodontic lesions persisting after failed endodontic treatment and endodontic surgery, and discusses the relationship between the procedure and periapical lesions. Surgical removal of hopeless teeth 11, 12 and 21 was performed conservatively in such a way to preserve the anatomy and gingival esthetics. A second surgical access was gained at the apical level, allowing the debridement of the surgical chamber for elimination of the periapical lesion, visual orientation for setting of the implants and filling of the surgical chamber with xenogenous bovine bone graft. After this procedure, the bone chamber was covered with an absorbent membrane and the healing screws were positioned on the implants. Later, a provisional partial removable denture was installed and the implants were inserted after 6 months. After 3 years of rehabilitation, the implants present satisfactory functional and esthetic conditions, suggesting that immediate implant placement combined with guided bone regeneration may be indicated for replacing teeth lost due to chronic periapical lesions with endodontic failure history in the anterior maxilla.


A integridade estética/funcional do tecido periodontal pode ser comprometida pela perda do elemento dental. Os implantes imediatos tornaram-se uma opção viável na manutenção da arquitetura periodontal, desde que haja a compatibilidade com o alvéolo e a possibilidade da contaminação local ser totalmente eliminada. O objetivo deste estudo foi descrever o procedimento de instalação de implantes imediatos na região anterior de maxila usados na reposição de dentes com lesões crônicas periapicais, discutindo a relação entre o procedimento e lesões periapicais. A remoção dos dentes condenados (11, 12 e 21) foi feita de forma conservadora a fim de preservar a estética e arquitetura periodontal. Um segundo acesso cirúrgico foi obtido a nível apical, permitindo: o debridamento da loja cirúrgica, orientação visual dos implantes e o preenchimento com enxerto ósseo bovino. A loja óssea foi coberta por uma membrana absorvível e uma prótese parcial removível provisória foi instalada e após 6 meses os implantes foram reabilitados. Após 3 anos de reabilitação, os implantes se apresentam em condições satisfatórias de estética/função, sugerindo que a instalação de implantes imediatos combinados com a regeneração tecidual guiada em lesões periapicais crônicas podem ser indicados para repor dentes perdidos devido a lesões periapicais crônicas com história de insucesso endodôntico na maxila anterior.

ABSTRACT

Duzentos e oitenta leitões, entre 40 e 70 dias de idade, afetados pela sindrome multissistêmica do definhamento (SMD) foram examinados após eutanásia para pesquisa de lesões dentárias. Pelo menos um abscesso periapical foi observado em 58 leitões (20,7 por cento), dos quais 12 apresentaram abscessos múltiplos. Terceiros incisivos superiores, 3os incisivos inferiores, caninos superiores, caninos inferiores e outros dentes apresentaram respectivamente 22 (31,4 por cento), 16 (22,9 por cento), 4 (5,7 por cento), 23 (32,9 por cento) e 5 (7,1 por cento) abscessos periapicais. A maior prevalência de abscessos observada nos dentes 3os incisivos e caninos inferiores está provavelmente associada com a maior área de corte ou desgaste que sofrem esses dentes. Entre as bactérias isoladas de amostras de 65 abscessos, Streptococcus sp. foi a mais prevalente e esteve presente em 21,48 por cento e 27,7 por cento dos isolados em aerobiose e anaerobiose, respectivamente. Na segunda posição em prevalência estiveram as bactérias corineformes, as quais foram mais freqüentemente isoladas em atmosfera anaeróbica do que aeróbica. Houve preponderância de isolamentos de bactérias Gram-positivas. Não foi possível determinar se as co-infecções bacterianas predispuseram às lesões características de SMD ou foram conseqüentes à imunossupressão causada pela infecção com PCV2.


Swine producers have historically resected needle teeth of newborn pigs to prevent potential injuries to the sow mammary gland and faces of littermates. However, the possible impact of this practice on pig performance has been questioned. As part of a study, 280 PMWS affected piglets, with 40-70 days of age, were examined for the presence of dental lesions immediately after euthanasia. Most pigs were confirmed as PMWS by the detection of typical microscopic lesions and immunohistochemical pattern. At least one periapical abscess was observed in 58 piglets (20.7 percent), of which 12 had multiple abscesses. There were 22 (31.4 percent), 16 (22.9 percent), 4 (5.7 percent), 23 (32.9 percent), and 5 (7.1 percent) abscesses in 3rd upper incisives, 3rd lower incisives, upper canines, lower canines, and other teeth, respectively. The higher prevalence of dental abscesses observed in 3rd incisives and lower canines is probably associated with the wider area of grinding or clipping to which these teeth were subjected. Among bacteria isolated from samples collected from 65 abscesses, Streptococcus sp. was the most prevalent bacteria and was present in 21.48 percent and 27.7 percent of the aerobic and anaerobic isolates, respectively. In the second most frequent group were coryneform microorganisms, which showed higher rate of isolation under anaerobic atmosphere than in aerobic culture. There was preponderance of Gram-positive isolates. It could not be determined whether these bacterial co-infections predisposed pigs to development of PMWS-type lesions or if were subsequent to the PCV2 infection.

A prevalência das patologias periapicais é um assunto de discordância entre investigadores. A variação dos achados pode ser atribuída à falta de padronização do material coletado, a diferentes técnicas de processamento do espécime e aos critérios de diagnóstico histológico utilizados por cada investigador. Este trabalho teve como objetivo re-avaliar casos de abscessos periapicais através de cortes semi-seriados. Foram selecionados casos de diagnóstico histopatológico de abscessos periapicias do Laboratório de Patologia Bucal da FOPUCRS, no período de abril/01 a setembro/2002 (18 meses). Os respectivos blocos de parafina foram localizados no arquivo e três novos cortes semi-seriados (com um espaçamento de 20mm entre cada corte) foram realizados. Os cortes foram corados com hematoxilina e eosina e avaliados por dois examinadores. Dos 23 casos de abscessos diagnosticados por ambos avaliadores, 18 (78,3 porcento) mantiveram o diagnóstico inalterado, enquanto que, cinco (21,7 porcento) alteraram o diagnóstico para cisto. Com base na metodologia empregada nesse experimento, concluímos que a utilização de cortes semi-seriados se faz necessária para o correto diagnóstico das lesões inflamatórias periapicais. Entretanto, a utilização desse recurso não minimizará as diferenças existentes entre os estudos de prevalência das lesões inflamatórias periapicais, uma vez que, ainda existe discrepância de critérios de diagnóstico histopatológicos entre os investigadores

ABSTRACT

Dental abscess is considered as one of the most common complications of untreated caries in children. Microorganisms are known as major causes for the pulp and periradicular pathosis. Black - pigmented, gram -negative anaerobes, belonging to species of porphyromonas and prevotella [bacteroides] play an important role in causing clinical signs and symptoms related to pulp and periradicular disease. The aim of this investigation was to assess the presence of two main pathogenic microorganisms named porphyromonas gingivalis and prevotella melaninogenica in abscessed primary teeth. Forty children aged 4-10 years were selected with no systemic disease and no antibiotic consumption during the last two weeks of sampling. Selected teeth were presenting swelling and pain at observation stage as signs of acute abscess. The diagnostic technique of porphyromonas gingivalis and prevotella melaningenica was the use of DNA formulation reading by means of PCR technique. Results showed that of the 40 samples, 35 [%87.5] were positive for porphyromonas gingivalis, 34 [%85] were positive for prevotella melaninogenica. However, it seems that more work is required for more precise antibiotic therapy of dental abscess in children. O objetivo deste estudo foi o de investigar a distribuição de linfócitos CD8+ e CD20+ em lesões inflamatórias periapicais. Para tanto foram estudados 90 casos entre abscessos crônicos, cistos abscedados e cistos inflamatórios. A tecnica de imunohistoquímica pelo método da estreptavidina-biotina foi utilizada para identificar linfócitos T citotóxico/supressor (CD8) e linfócito B (CD20). Dentre os resultados encontrados notou-se uma distribuição das células CD8+ da seguinte forma: 1) difusa na capsula fibrosa dos abscessos crônicos (58,8 por cento) e ausente nos cistos abscedados (64,1 por cento) e cistos inflamatórios (70,6 por cento); 2) zona infiltrativa: difusa nos cistos abscedados (100 por cento) e cistos inflamatórios (82,4 por cento); 3) zona subepitelial: ausente nos cistos inflamatórios (53,0 por cento) e difusa nos cistos abscedados (56,4 por cento); 4) zona de supuração: difusa nos abscessos crônicos (100 por cento) e cistos abscedados (97,5 por cento). As células CD20+ apresentavam a seguinte distribuição: 1) cápsula fibrosa: ausente nos cistos inflamatórios (100 por cento), cistos abscedados (94,8 por cento) e abscessos crônicos (88,3 por cento); 2) zona infiltrativa: difusa nos cistos abscedados (100 por cento) e cistos inflamatórios (53 por cento); 3) zona subepitelial: ausente nos cistos inflamatórios (58,8 por cento) e focal nos cistos abscedados (46,2 por cento); 4) zona de supuração: difusa nos cistos abscedados (100 por cento) e abscessos crônicos (100 por cento). Em conclusão é possível afirmar que a distribuição linfocitária é predominantemente difusa para ambos os tipos de linfócitos.

ABSTRACT

En este estudio, se reportó la respuesta del tejido periapical ante la exposición prolongada del tejido pulpar y el efecto de la administración de un antiinflamatorio no esteroide (piroxicam), sobre dos tipos celulares involucrados en la respuesta inflamatoria e inmunológica local como son las células cebadas y los basófilos, utilizando ratas Long Evans a las cuales se les provocó exposición pulpar previo a la administración del piroxicam. Los hallazgos fueron: en el área de los abscesos en el grupo experimental, a los 8 días las células cebadas se encontraron en un porcentaje del 50.0 disminuyendo a 16.6 por ciento a los 15 días, mientras que en el grupo control a los 8 días se presentaron en 11.1 por ciento, aumentando a 22.2 por ciento a los 15 días en el total de los animales. Con respecto a los basófilos en la misma zona se observó en el grupo control a los 8 días un porcentaje de 61.1 disminuyendo a 33.3 por ciento a los 15 días, siendo lo contrario en el grupo experimental, donde de 16.6 por ciento aumentó a 33.3 por ciento en el transcurso del tratamiento; la degranulación fue raramente observada en ambos tipos celulares probablemente por efecto del antiinflamatorio en estudio
